By your General Durable Power of Attorney, you provide him/her with the authority to manage your financial affairs in the event that you are unable to do so. Under this document, the person named has the ultimate decision - making authority with respect to your financial affairs.
Health Care Instruction Directive (Living Will)
The proposed Health Care Instruction Directive outlines your wishes concerning life sustaining treatment in the event that you are unable to communicate your wishes. Under this document, you direct that you do not want to accept medical treatment by artificial means if it is determined that you are in a condition where there is no prognosis of recovery.
Health Care Proxy Directive
By your Health Care Proxy Directive, you provide him/her with the authority to make health care decisions, consistent with your Health Care Instruction Directive, in the event that you are unable to do so.
